COVID 19 Precautions

Note, as an added precaution due to the COVID19 pandemic, in order to reduce wait times in our front office area, we are asking all new patients to complete their initial intake paperwork prior to their first appointment. Additionally, we ask at this time that all patients and staff wear face masks upon entering our building. If you do not have any available, we are more than happy to provide a covering for the length of your appointment.

Finally, in order to best utilize the time of our administrative staff, we ask patients arrive no earlier than 10 minutes before the start of their appointment. Upon arrival, please call 206.588.0855 and our admin staff will be happy to run down to perform our health screening questionnaire.***

Union Physical Therapy has a 24 hour cancellation / rescheduling policy.

If you miss your appointment, cancel, or change your appointment with less than 24 business hours’ notice, you will be charged $50. If you no-show or late cancel for 2 or more visits Union PT reserves the right to discharge you from our care.
